
Regulation requires that Limestone Surgery Center notify its patients regarding the policy on Advance Directives in advance of the date of surgery.
Although, it is not required that you have a living will or advance directive, you have the right to have one and the right to have the living will placed in your medical record.
The Limestone Surgery Center does not honor Advance Directives/Living Wills.
If you desire to submit an Advance Directives (e.g. living will) please provide it upon arrival. If the directive contains a do not resuscitate provision, the provision will be suspended during your surgical procedure and recovery in the facility. If you are transferred to a more acute level of care, such as a hospital emergency room and you have provided us with an Advance Directive/Living Will, it will be sent with you along with other parts of your medical record.
